A PRIVACY PLEDGE TO OUR CREDIT UNION MEMBERS
Your credit union wants you to know about the information it collects about you, and about how this information might be used.
Information We Collect
We collect nonpublic information about you from some or all of the following sources:
-Information we receive from you on applications or other forms, such as your name, address, social security number, assets and income;
-Information about your transactions with us, our affiliates, or others, such as your account balance, payment history, parties to transactions and credit card usage; and,
-Information we receive from a consumer reporting agency, such as your creditworthiness and credit history.
Information We Disclose
We may disclose all of the nonpublic information about our former members we collect as described above.
Who May Receive This Information
We may disclose the information we collect as described above to provide high quality products and services to you. We may disclose the information to the following types of third parties:
-Financial services providers such as mortgage bankers, securities brokers/dealers, and insurance agents.
Our Security Measures
We restrict access to nonpublic information about you to those employees who need to know that information to provide products or services to you. We maintain physical, electronic and procedural safeguards that comply with Federal regulations to guard your nonpublic personal information.
You May Opt Out
If you prefer that we do not disclose nonpublic information about you to nonaffiliated third parties, you may opt out of those disclosures, that is, you may direct us not to make those disclosures (other than disclosures permitted by law). Non-member joint account holders, coborrowers and guarantors may exercise this right to opt out. For joint account holders, we will treat an opt-out request as applying to everyone listed on the account unless you direct us otherwise. If you wish to opt out of our disclosures to non-financial companies such as retailers, direct marketers, and publishers, or others such as non-profit organizations, you may call our toll free number, or mail your request to us as follows:

Online Banking Disclosure
We are Children’s Medical Center Federal Credit Union, hereinafter referred to as "we" and "us", located at 3333 Burnet Avenue., M/L 5010, Cincinnati, OH 45229 and our phone number is (513) 636-4470.
"You" and "your" refers to the member-owner of a savings account who has entered into an agreement relating to Children’s Medical Center FCU Online Banking. You hereby agree to the rules and regulations affecting the issuance of the Online Banking User ID and Password for the Children’s Medical Center FCU Online Banking service provided by us for your convenience.
User ID & Password: The User ID & Password you choose is your "remote banking signature", and you are responsible for maintaining its confidentiality. If you choose to give your User ID & Password to someone else, you are responsible for any and all transactions that may occur on your accounts that are connected to the Online Banking system.
Authorized Use: Only you are authorized to withdraw funds from your account(s) with the use of your Online Banking User ID and Password along with any joint owner you may share your username and password with.
Notification Procedure: If you believe that your Online Banking Password has been LOST or STOLEN, or that someone has transferred or may transfer money from your account without permission, call us at the number shown at the beginning of this Agreement, or write us at the address given at the beginning of this Agreement.
Customer Service Office Hours for Online Banking: Our customer assistance number is available during normal business hours of operation (See Credit Union hours for details). The number is 513-636-4470.
Types of Transactions Available: You may use your Online Banking Password to obtain account information related to any of your savings and loan accounts regarding current balances and account history; savings dividend rates; loan interest rates; payoff amount;, payments and due dates; and YTD dividend earned and interest paid on each account. You can change passwords and export history.
Transfers: You may make deposit transfers to your accounts or other accounts you authorize as often as you like. You may transfer up to the balance in your accounts at the time of the transfer, except as limited under other agreements. We reserve the right to refuse any transaction that would draw upon insufficient funds or lower an account below a required balance

Error Resolution for Online Banking: In case of errors or questions about your transactions, call us at 513-636-4470 for Online Banking or write us at the address listed in this Agreement, as soon as you can, if you think your statement is wrong or if you need more information about a transfer listed on the statement. We must hear from you no later than 60 days after we send the FIRST statement on which the problem or error appeared. (1) Tell us your name and account number (2) Describe the error or the transfer you are unsure about, and explain as clearly as you can why you believe it is an error or why you need more information. (3) Tell us the dollar amount of the suspected error.
If you tell us orally, we may require that you send us your complaint or question in writing within 10 business days. We will tell you the results of our investigation within 10 business days after we hear from you and will correct any error promptly. If we need more time, however, we may take up to 45 days to investigate your complaint or question. If we decide to do this, we will credit your account within 10 business days for the amount you think is in error, so that you will have the use of the money during the time it takes us to complete our investigation. If we ask you to put your complaint or question in writing and we do not receive it within 10 business days, we may not credit your account. If we decide that there was no error, we will send you a written explanation within 3 business days after we finish our investigation. You may ask for copies of the documents that we used in our investigation.
Cancellation: Your Online Banking User ID & Password remains our property. We may cancel your Online Banking privileges at any time without notice or cause. Any cancellation or termination shall not affect any of your existing liability to us.
Liability: If we do not complete a transfer to or from your account on time or in the correct amount according to our agreement with you, we will be liable for your losses or damages. However, there are some exceptions. We will not be liable for:
If, through no fault of ours, you do not have enough money in your account to make the transfer.
If the Online Banking System was not working properly and you knew about the breakdown when you started the transfer.
If circumstances beyond our control (such as fire or flood) prevent the transfer, despite reasonable precautions that we have taken.
Conditions Under Which We Will Disclose Information To A Third Party: You agree that we may, and you hereby authorize us to, disclose information to third parties about your account(s) or the transfers you make (1) where it is necessary for completing transfers; (2) in order to verify the existence and condition of your account for a third party, such as a credit bureau or merchant; (3) in order to comply with government agency or court orders as permitted by law; or (4) if you give us your written permission.
